５日午後０時４８分ごろ、大阪府吹田市の万博記念公園内にある遊園地「エキスポランド」で、ジェットコースター「風神雷神２」（６両編成、全長１０５０メートル、最高時速７５キロ）の２両目が脱線し、同車両の前列左側に乗っていた女性が鉄製の手すりと衝突し死亡した。２両目前部の車軸部分が折れており、走行中に車体がレールから外れて、最大で４５度傾いて走行した。死亡した女性と乗客１９人と、事故を見て気分が悪くなった人を含む計３４人が病院に搬送された。コースターは毎日、車両本体や安全装置を点検し、空の状態で３回運行させている。この日の朝の点検で、異常は発見されなかった。午後１時半から３０分の中間点検が予定されていたが、その直前に事故が発生した。
